3. ELIGIBILITY

The G&L HBM 2025 Health Check Giveaway is open to legal residents of the United States who are at least 18 years old at the time of entry. Employees of the Fives MTSS, its affiliates, subsidiaries, and immediate family members are not eligible for the HBM 2025 Health Check Giveaway. The HBM 2025 Health Check Giveaway is void where prohibited by law.
4. GIVEAWAY PERIOD
The HBM 2025 Health Check Giveaway begins on 4/7/25 at 8:00am EST and ends on 12/23/25 at 5:00pm EST (the “Entry Period”). Entries submitted outside this period will not be accepted.
5. HOW TO ENTER
Participants may enter the HBM 2025 Health Check Giveaway in one of the following ways:
- Purchase Method: Each time you place an order with Fives MTSS during the Entry Period, you can receive one entry in the giveaway. To qualify, you must indicate at the time of purchase that you would like to be entered into the HBM 2025 Health Check Giveaway. Must provide qualified Giddings & Lewis Horizontal Boring Mill serial number. The HBM 2025 Health Check Giveaway Code “HBM2025” will be added to your order, and each order with the code will be counted as a separate entry for that month’s drawing.
- Phone Orders – Mention the HBM2025 Promo Code on all qualifying HBM orders
- Online Orders – Enter HBM2025 in the Promo Code field on all qualifying HBM orders
- Email Orders – Include in the Subject Line Promo Code HBM2025 on all qualifying HBM orders

All entries must be received by the last day of the month to be eligible for the next drawing. Offer valid for qualifying purchases made during the promotional period. 6. PRIZE DETAILS (Monthly Winners)

*Included in Health Check: During the Health Check Service, the following scope of work will be performed. A review of the work will be presented prior to the technician leaving your facility, any items not present or operable at the time of service will be identified. Service includes the following where applicable and accessible. VISUALLY INSPECT, CHECK & ANALYZE: Machine E-Stop functionality & safety check, Linear ways or guides & assess bearings condition, Linear axes bearing wipers condition, Ball screws or drive racks condition, Check Axes for lost motion, Hydraulic power system condition and pressures, Spindle(s); verify mist/lube, runout & heat checks, Chiller unit condition & function, Coolant and Chip Removal (where applicable), Machine electrical functionality & condition, Machine wiring condition & connections check, Shuttle & pallet stands inspection (if applicable), Handheld pendant function check, Tool changer function check (if applicable), Overall machine performance review, Machine guarding, enclosure & interlocks checks, Axes covers, bellows or way cover condition, Way & gibs Rulon/Turcite wear material condition, Gearbox, drive belts & couplings condition, Machine axes counterbalance check (if applicable), Pneumatic system condition and pressures, Spindle(s) visual checks, Clamping force checks, Check machine lube oil, grease & filters, Controller condition / shutdown & power up check, Power tracks, cabling, conduit & hoses/tubing, Pallet changer & pallet condition (if applicable), Tool / part probe function check, Machine data backup, Follow up recommended parts and service report. MACHINE LEVEL & ALIGNMENT CHECKS: Machine geometries will be checked utilizing the conventional method of squares and indicators, or the Renishaw ballbar system. This quotation does not include the re-leveling or scraping of machine alignments. Alignment corrections requiring re-leveling, or scraping are available at additional cost.
7. WINNER SELECTION & NOTIFICATION
The winner(s) will be selected within 10 days of the end of each month randomly from all eligible entries. The winner will be notified via email within 7 days. The winner must respond within 14 days to claim the prize. If the winner does not respond within this timeframe, the Sponsor may select an alternate winner.
8. PUBLICITY RELEASE
By entering, participants grant the Sponsor the right to use their name, likeness, and entry for promotional purposes without additional compensation, except where prohibited by law.
9. GENERAL CONDITIONS
The Sponsor is not responsible for lost, late, incomplete, misdirected, or undelivered entries, nor for any technical issues that may affect participation. The Sponsor reserves the right to cancel, modify, or suspend the Giveaway if fraud, technical failures, or any other factors beyond its control compromise the integrity of the Giveaway.
10. LIMITATION OF LIABILITY
By entering, participants release and hold harmless the Sponsor and its affiliates from any claims, damages, or liability arising from participation in the Giveaway or acceptance of any prize.
11. PRIVACY POLICY
Any personal information collected during the Giveaway will be used solely for the administration of the Giveaway and will not be shared or sold to third parties.
12. GOVERNING LAW
This Giveaway is governed by the laws of the state of Kentucky without regard to its conflict of law principles.
13. ODDS OF WINNING
The odds of winning are based on the number of eligible entries received.
